Job Description

LPN (remote)

Primary Responsibilities
•    Expectation to meet KPI (50 + calls per shift)
•    Assess patient's health status and recommend care based on clinical judgment and protocols
•    Identify potential care and/or provider gaps
•    Coach patients on treatment alternatives 
•    Coordinate services and referrals to health programs and community services
•    Assesses and triage immediate prescription requests
•    Initiate the plan of treatment/care pathway based upon the analysis of information obtained and within scope of practice and certifications
•    Accurately communicate (written/verbal) patient status information to prescribing physician and case manager in a timely manner and provide necessary clinical information to pharmacists and other healthcare professionals as applicable
•    Provide care and services to assigned caseload in an efficient, organized manner

 

Required Qualifications
• Current, unrestricted RN/LPN licensure 
• 2+ years of clinical experience
• Proven track record of excellent Clinical record documentation
 
 

Preferred Qualifications
•    1 year of phone experience/remote
•    Basic knowledge of theories, practices and procedures in a function or skill
•    Perform routine or structured work
•    Respond to routine or standard requests
 

Expected annual pay for this role ranges from $33,000 to $76,000 . Based on the position, the role is also eligible for Wipro’s standard benefits including a full range of medical and dental benefits options, disability insurance, paid time off (inclusive of sick leave), other paid and unpaid leave options.
